Healthy Together
A regional initiative improving health outcomes for patients with heart disease and substance use disorder in Allegany and Garrett County.
AHEC West leads the Healthy Together program, funded by a $4.5 million grant from the Maryland Community Health Resource Commission's Health Equity Resource Communities (HERC) initiative. The program works across Allegany and Garrett County to improve care for patients managing heart disease or substance use disorder.
Healthy Together brings together a robust network of regional partners — health departments, social services agencies, and community health centers — each contributing to a shared set of goals around access, equity, and outcomes.
Healthy Together serves residents of Allegany and Garrett counties living with heart disease or substance use disorder, as well as providers and community organizations working to improve health equity in Western Maryland.

- Expanding access to primary care
- Issuing security deposit vouchers to enhance housing stability
